If you're having trouble verifying your details please check the following:
Enter your name as it appears in your Medicare card.
Initial is optional and may not appear on your card.
Medicare number appears above your name on the front of your card.
Individual reference number appears to the left of your name on the front of your card.
Expiry date appears next to 'Valid to' on the front of your card.
Please make sure your card is not expired
